Updating or Changing your Name or Address

Should you need to change your name (due to a legal name change) or update your address, you can easily make the changes once your ID has been updated.


To do so, access the Smart Heath app and follow these directions:



1. Click Profile

2. Click Contact Info

3. Click the > in the Address on ID box

4. Click Proceed to make updates to any changed information on your ID (IMPORTANT: you will  need your new ID to make any changes to your name or address)

5. Click Begin Verifying - Follow the on-screen steps to scan your new ID with the changed details.
